QIC-AG Continuum Assessment Tool: Element 3

This resource is an Adapted Assessment Tool originally developed by AdoptUSKids and customized by the QIC‑AG to support Element 3 of the QIC‑AG Continuum Assessment process. Delivered as an interactive Excel workbook, the tool helps child welfare system administrators and partners systematically evaluate the quality and accessibility of their existing service array for foster, adoptive, and guardianship families across the permanency continuum. It supports data‑informed analysis by identifying strengths and gaps in services, enabling systems to prioritize improvements that strengthen post‑permanency supports and promote long‑term family stability.
