QIC- AG Overview of the Permanency Survey (Vermont)

This resource describes Vermont’s implementation of the Vermont Permanency Survey as part of the Quality Improvement Center for Adoption and Guardianship Support and Preservation (QIC‑AG). Implemented within the Universal interval of the QIC‑AG Permanency Continuum Framework, the project uses statewide, proactive outreach to engage all adoptive and guardianship families and gather information on family, child, and caregiver well‑being, as well as access to community services.
