Overview

This resource contains the finalized Vermont Permanency Survey used during Cycle 3 of Vermont’s QIC‑AG project to gather detailed information directly from adoptive and guardianship families. The survey instrument includes validated measures and structured questions focused on family well‑being, child well‑being, caregiver well‑being, adoption and guardianship experiences, and access to community services. Designed to support universal outreach, the survey helps Vermont systematically identify family strengths, monitor post‑permanency needs, and provide families with opportunities to connect to appropriate supports, while generating data to inform statewide planning and continuous improvement of post‑adoption and guardianship services.
